Document Checklist for Star Card

PHYSICAL COPIES ONLY

Proof of Identity - You will need ONE of the following:

• Valid, Unexpired, U.S. Passport or U.S. Passport Card
• State Certified Birth Certificate **
• Consular Report of Birth Abroad
• Certificate of Citizenship
• Unexpired Permanent Resident Card
• Unexpired Employment Authorization Document with Unexpired Foreign Passport
• Unexpired Foreign Passport with U.S. Visa and I-94

** If your name is different than on your proof of identity document, (Birth Certificate) we will need any name change documents such as: Certified Marriage License of Divorce Decree

Social Security Number - You will need ONE of the following:

• Social Security Card
• W-2 of Form 1099
• Paystub with full social security number

Proof of Idaho Residency - You will need TWO of the following

• Utility Bill with Current Physical Address (CELL PHONE BILLS NOT ACCEPTED)
• Bank or Credit Card Statement (FULL STATEMENT NOT JUNK MAIL FROM BANK)
• Medical or Dental Bill
• Paycheck Stub, or Employment Records
• Military Leave and Earning Statement
• Idaho School Enrollment Records (NO COLLEGE)
• Record of Property Taxes Assessed/Paid and Within 1 Year
• Valid Vehicle Insurance that has Physical Address and is from an Idaho Based Carrier
